What is oppositional defiant disorder?

Oppositional defiant disorder (ODD) is a childhood behavioral condition marked by an ongoing pattern of angry or irritable mood, argumentative or defiant behavior, and acting out of spite, often toward parents, teachers and other authority figures, lasting at least six months.

All children push back sometimes. With ODD, the behavior is more frequent and intense than expected for the child’s age and causes real problems at home, at school or with friends. ODD often occurs alongside ADHD, anxiety, depression or learning differences, and treating those can make a big difference.

When to seek help

If defiance and anger are affecting your child’s school, friendships or family life, or you feel stuck in daily battles, an evaluation can help. Early support often prevents problems from growing as children get older.

Common signs of ODD

These patterns are persistent, happen with more than one person, and go beyond typical testing of limits.

  • Frequent temper outbursts or loss of temper
  • Often touchy, easily annoyed or resentful
  • Arguing with adults and refusing to follow rules
  • Deliberately annoying others
  • Blaming others for mistakes or misbehavior
  • Acting out of spite or wanting to get back at others

How we treat ODD

ODD care is a partnership with families. We look at the whole child and give parents practical support, not blame. Our provider brings 12 years of pediatric nursing experience to every visit.

  1. A thorough psychiatric evaluation

We meet with your child and with you, review behavior at home and school, and check for ADHD, anxiety, mood or learning concerns that may be driving the behavior.

  2. Treating related conditions

When ADHD, anxiety or depression is part of the picture, treating it, including with medication when appropriate, often improves behavior too.

  3. Family and school coordination

Parent training and family therapy are core treatments for ODD. With your permission, we coordinate with therapists, schools and pediatricians so everyone is aligned.

  4. Ongoing support

Regular follow-up visits, in person in Bellevue or by telehealth anywhere in Washington State, so we can track how your child is doing and adjust the plan as you go.

Oppositional Defiant Disorder FAQs

What causes oppositional defiant disorder?

There’s no single cause. ODD likely comes from a combination of a child’s temperament, brain-based differences, and environmental stress. It often occurs alongside ADHD, anxiety or learning differences. It is not caused by bad parenting.

Is there medication for ODD?

There’s no medication approved specifically for ODD. When a related condition such as ADHD, anxiety or depression is present, treating it with medication can significantly improve behavior. Parent training and therapy are the main treatments for ODD itself.

Will my child grow out of ODD?

Many children improve, especially with early support. Without help, some continue to struggle as teens. Treatment gives your child and your family the best chance of things getting better.

How is ODD diagnosed?

Through a careful evaluation that includes talking with your child and with you, and often, with your permission, input from teachers. We look at how long the behavior has lasted, how often it happens and whether other conditions are involved.

Do parents take part in treatment?

Yes. Parents and caregivers are essential partners for children, and for teens, when appropriate and with your teen’s agreement. We include you in the evaluation and planning, and coordinate parent training or family therapy when it’s recommended.

How long is the first appointment for ODD?

The first psychiatric evaluation takes about 60 minutes. Follow-up visits take about 30 minutes, in person in Bellevue or by telehealth.

What should I bring to my child’s first appointment?

Please bring your photo ID, your child’s insurance card and a list of their current medications. If you have them, report cards, notes from school and any earlier evaluations help too.

Do you offer ODD care by telehealth?

Yes. Evaluations and follow-up visits are available by secure telehealth anywhere in Washington State, as well as in person at our Bellevue office.

Is ODD treatment covered by insurance?

We’re in-network with most major insurance plans, including Medicare, and self-pay is available. Before your first visit, our team verifies your coverage.
