What is a personality disorder?

A personality disorder is a long-lasting pattern of thinking, feeling and relating to others that differs from what’s expected, is inflexible across situations, and causes distress or problems in relationships, work or daily life. These patterns usually begin in adolescence or early adulthood.

There are several types. Borderline personality disorder (BPD), which involves intense emotions, fear of abandonment, unstable relationships and impulsive behavior, is one of the most common in outpatient care. Personality disorders often occur alongside depression, anxiety, PTSD or substance use.

When to seek help

If the same painful patterns keep repeating in your relationships or emotions, or you’re struggling with impulsive or self-harming behavior, reach out. If you’re in crisis or thinking about harming yourself, call or text 988 or call 911.

Common signs of a personality disorder

Signs vary by type. These patterns are ongoing and show up in many areas of life, not just during stressful times.

  • Intense, fast-changing emotions that are hard to manage
  • Relationships that often feel unstable or conflict-filled
  • Strong fear of rejection or abandonment
  • An unclear or shifting sense of who you are
  • Impulsive or self-destructive behavior
  • Persistent distrust, detachment or difficulty connecting with others

How we treat personality disorders

Treatment is a collaboration built on trust. We focus on what’s causing you the most distress and build on your strengths.

  1. A thorough psychiatric evaluation

We take a careful, unhurried history to understand your patterns, your relationships and any related conditions, such as depression, anxiety, PTSD or ADHD.

  2. Targeted medication management

There’s no single medication for personality disorders, but medication can help with specific symptoms such as mood swings, anxiety, depression or impulsivity.

  3. Therapy coordination

Specialized therapies such as dialectical behavior therapy (DBT) are the core treatment for many personality disorders. We coordinate with therapists so your care works together.

  4. Ongoing support

Regular follow-up visits, in person in Bellevue or by telehealth anywhere in Washington State, so we can track how you’re doing and adjust your plan as you go.

Personality Disorders FAQs

Can personality disorders be treated?

Yes. Personality disorders were once thought to be hard to treat, but research shows many people improve significantly. Borderline personality disorder in particular often responds well to specialized therapy, with many people no longer having the condition over time.

Is there medication for personality disorders?

No medication is approved specifically for personality disorders, but medication can help with symptoms such as depression, anxiety, mood instability and impulsivity. It works best alongside therapy.

What is borderline personality disorder?

Borderline personality disorder (BPD) involves intense and shifting emotions, fear of abandonment, unstable relationships, an unsteady sense of self and impulsive behavior. Dialectical behavior therapy (DBT) is one of the most effective treatments.

How is a personality disorder diagnosed?

A personality disorder is diagnosed through a thorough psychiatric evaluation, sometimes over more than one visit. We look at long-term patterns rather than how you feel during a single difficult period, and consider other conditions that can look similar.

Will I be judged for having a personality disorder?

No. A diagnosis is simply a way to understand what you’re experiencing and guide treatment. Our team provides respectful, compassionate care without judgment.

Can teenagers be diagnosed with a personality disorder?

Personality patterns often first appear in adolescence, and some conditions, such as borderline personality disorder, can be diagnosed in teens when the patterns are long-lasting. Because teens are still developing, we make that call carefully. We provide psychiatric care for adolescents as well as adults, with parents or caregivers involved in the plan.

What’s the difference between borderline personality disorder and bipolar disorder?

Both involve big shifts in mood, so they can look alike. In bipolar disorder, moods change in distinct episodes that last days or weeks, with steadier periods in between. In BPD, emotions shift quickly, often within hours, and are often set off by stress in relationships. The two can also occur together, which is why a careful evaluation matters.

How long is the first appointment for a personality disorder?

The first psychiatric evaluation takes about 60 minutes. Follow-up visits take about 30 minutes, in person in Bellevue or by telehealth.

What should I bring to my first appointment?

Please bring a photo ID, your insurance card and a list of current medications. If you’ve had earlier evaluations or treatment, including therapy such as DBT, those records help too.

Do you offer personality disorder treatment by telehealth?

Yes. Evaluations and follow-up visits are available by secure telehealth anywhere in Washington State, as well as in person at our Bellevue office.

Is personality disorder treatment covered by insurance?

We’re in-network with most major insurance plans, including Medicare, and self-pay is available. Before your first visit, our team verifies your coverage.
