What is PMDD?

Premenstrual dysphoric disorder (PMDD) is a severe form of premenstrual syndrome that causes significant mood symptoms, such as depression, irritability, anxiety and mood swings, in the week or two before a menstrual period. Symptoms ease within a few days after the period starts.

PMDD is thought to come from an unusual sensitivity in the brain to the normal hormone changes of the menstrual cycle, not from abnormal hormone levels. It’s a recognized depressive disorder and is very treatable.

When to seek help

If your premenstrual mood changes affect work, school or relationships, or you notice a cycle of feeling fine and then struggling every month, an evaluation can help. Tracking your symptoms for two cycles before your visit is useful but not required.

Common signs of PMDD

PMDD follows a clear monthly pattern: symptoms appear before your period and fade soon after it begins.

  • Marked sadness, hopelessness or tearfulness
  • Strong irritability, anger or conflict with others
  • Anxiety, tension or feeling on edge
  • Sudden mood swings or feeling overwhelmed
  • Low energy, trouble concentrating or changes in sleep
  • Physical symptoms such as bloating, breast tenderness or cravings

How we treat PMDD

PMDD treatment focuses on the mood symptoms that disrupt your life, with a plan that fits your cycle and your goals.

  1. A thorough psychiatric evaluation

We review the timing and pattern of your symptoms, your history and other conditions, such as depression or anxiety, that can worsen before a period.

  2. Medication management

Certain antidepressants are well studied for PMDD and can be taken daily or only during the second half of the cycle. We help you find what works.

  3. Coordinated care

With your permission, we coordinate with your gynecologist or primary care provider, especially if hormonal treatment is part of your plan.

  4. Ongoing support

Regular follow-up visits, in person in Bellevue or by telehealth anywhere in Washington State, so we can track how you’re doing and adjust your plan as you go.

PMDD FAQs

What’s the difference between PMS and PMDD?

PMS involves mild to moderate physical and emotional symptoms before a period. PMDD is much more severe, with mood symptoms such as depression, anger or anxiety that significantly disrupt work, school or relationships.

How is PMDD diagnosed?

Diagnosis is based on the pattern of your symptoms: they appear in the week before your period, improve within a few days after it starts, and are minimal the week after. Tracking symptoms daily for at least two cycles helps confirm it.

What is the best treatment for PMDD?

Certain antidepressants (SSRIs) are a first-line treatment and often work quickly for PMDD. Some people take them only in the second half of their cycle. Hormonal options and lifestyle changes can also help.

Can PMDD get worse over time?

PMDD symptoms can change with life stages, such as after pregnancy or approaching perimenopause. Treatment can be adjusted as your needs change.

Do you offer PMDD treatment by telehealth?

Yes. We offer secure telehealth visits anywhere in Washington State, as well as in-person appointments at our Bellevue office.

Is PMDD a mental health condition?

Yes. PMDD is classified as a depressive disorder in the DSM-5, the manual clinicians use to diagnose mental health conditions, even though its timing follows the menstrual cycle.

Should I see a mental health provider or a gynecologist for PMDD?

Either can help, and many people benefit from both. A mental health provider focuses on the mood symptoms; at our clinic, that includes medications such as SSRIs. A gynecologist or primary care provider can discuss hormonal options. With your permission, we coordinate with your other providers.

Can teenagers have PMDD?

Yes. PMDD can begin any time after periods start, including the teen years. We provide psychiatric care for adolescents as well as adults, with parents or caregivers involved in the plan.

How long is the first appointment for PMDD?

The first psychiatric evaluation takes about 60 minutes. Follow-up visits take about 30 minutes, in person in Bellevue or by telehealth.

What should I bring to my first appointment?

Please bring a photo ID, your insurance card and a list of current medications. If you’ve tracked your symptoms, or had earlier evaluations or treatment for your mood, bring those notes or records too.

Is PMDD treatment covered by insurance?

We’re in-network with most major insurance plans, including Medicare, and self-pay is available. Before your first visit, our team verifies your coverage.
