Understanding Treatment-Resistant Depression
What is treatment-resistant depression?
Treatment-resistant depression (TRD) is major depression that hasn’t improved enough after trying at least two antidepressant medications at an adequate dose and for long enough. It is common: a significant share of people with depression don’t get full relief from their first treatments.
Having TRD doesn’t mean depression can’t be treated. It means a different approach is needed. Options include adjusting or combining medications, NeuroStar® TMS Therapy and SPRAVATO® (esketamine), and a careful review to make sure nothing else, such as bipolar disorder, thyroid problems or sleep issues, is getting in the way.
When to seek help
If you’ve tried at least two antidepressants and are still struggling, it’s a good time to explore advanced options. If you’re having thoughts of suicide, call or text 988 or call 911 right away.

Questions
Treatment-Resistant Depression FAQs
What counts as treatment-resistant depression?
Depression is generally considered treatment-resistant when it hasn’t improved enough after at least two antidepressants, each taken at an adequate dose for an adequate length of time.
What are the treatment options for treatment-resistant depression?
Options include medication changes or combinations, NeuroStar® TMS Therapy, SPRAVATO® (esketamine), and therapy. We offer evaluation, medication management, TMS and SPRAVATO® at our Bellevue clinic.
Is TMS or SPRAVATO® better for treatment-resistant depression?
Both are effective, and the right choice depends on your history, preferences, health and insurance. TMS is drug-free with daily short sessions; SPRAVATO® is a supervised nasal spray treatment. Your evaluation will help you decide.
Does insurance cover TMS and SPRAVATO®?
Many insurance plans, including Medicare, cover NeuroStar® TMS and SPRAVATO® for treatment-resistant depression when criteria are met. We verify your benefits and handle prior authorization for you.
Can I keep taking my antidepressant during TMS or SPRAVATO®?
Usually, yes. TMS can be used alongside medication, and SPRAVATO® is often taken together with an oral antidepressant. We’ll review your medications as part of your plan.
Can teenagers get TMS for treatment-resistant depression?
Yes. NeuroStar® Advanced TMS Therapy is FDA-cleared as an add-on treatment for depression in adolescents ages 15 to 21. Sessions take place at our Bellevue office. SPRAVATO® is for adults.
How long is the first appointment for treatment-resistant depression?
The first psychiatric evaluation takes about 60 minutes. Follow-up visits take about 30 minutes, in person in Bellevue or by telehealth.
What should I bring to my first appointment?
Please bring a photo ID, your insurance card and a list of current medications. A list of the antidepressants you’ve tried, with doses, how long you took them and any side effects, helps us plan next steps, along with records from earlier treatment.
Can I get care for treatment-resistant depression by telehealth?
Your evaluation and medication management visits are available by secure telehealth anywhere in Washington State, as well as in person in Bellevue. NeuroStar® TMS and SPRAVATO® treatments take place in person at our Bellevue office.
